Clients are connected to medical care through outreach & engagement services, as well as in interim housing and permanent supportive housing.

Our various outreach teams will partner with street medicine partners to bring medical care directly to our clients. At the Annenberg Access Center in Santa Monica, we are grateful to host Venice Family Clinic to provide medical care in two physicians’ rooms.
In interim housing we have operated innovative pilots to reduce our clients’ utilization of emergency rooms by strengthening their connections to primary healthcare. We are also proud to work with hospital partners funding interim housing beds to support patients experiencing homelessness after their discharge. Additionally, we operate enhanced care management (ECM) services, as part of CalAIM, to further support our clients receiving the healthcare they need.
SOLAR
The People Concern provides recuperative care services at our interim housing site, SOLAR, in Compton. With meals, full-time nursing care, case management, linkage to permanent housing and health and diet education, residents can stabilize after a hospital stay. Case managers and advocates work to place the program participants in permanent housing once they become stable.
Health and Wellness
Operating out of our interim housing facilities in Spa 4 and Spa 5, we assist chronically homeless individuals in improving their health by connecting them to essential health services. Our team guides them in navigating their insurance to manage their care once housed. Recently, we have expanded our support to the community through the ECM program with LA Care.
